Community Aged Care Packages
52 CACPs are managed from Wesley Home Care providing care to people in the near city and western region of Adelaide. Recipients of these packages must have a current Aged Care Assessment approval for Low Care.
These packages are planned and coordinated packages of care to help older people to remain living in their own homes. They are funded by the Federal Government, and have a mean-tested co-payment by the client. All C.A.C.P Care Plans are designed by the co-ordinator in conjunction with the client and their advocate. This allows and provides for flexibility in the design of each individual plan.
The Care Plans may include assistance with:
Personal Care such as:

showering, bathing or personal hygiene

Social support
-
Transport to appointments
-
Laundry
-
Meal preparation
-
Domestic support
-
Shopping and bill paying
-
Liasing with other service providers

Veterans Home Care
Funded by the Department of Veterans Affairs, Veterans Home Care provides support workers to assist with personal care, home help, respite, gardening and maintenance services. These services are designed to maintain Veterans, Veterans widows or carers of Veterans in their own home and prevent early admission to a Residential Aged Care Home. To access this service the enquiries must go through the Veterans Home Care Assessment Agency. Phone 1300 550 450.
Assistance with Care and Housing for the Aged
The aim of the ACHA program is to achieve a stable housing and support situation for financially disadvantaged older people who are renting, homeless or at risk of homelessness. This program is designed to meet their accommodation and support needs and to allow them to remain in the community thus avoiding premature entry into an aged care facility.
The ACHA program is funded by the Federal Government, Dept of Health and Ageing.
Community Visitors Scheme
This Federal Government funded program provides volunteers to visit, on a fortnightly basis, lonely and isolated residents in nursing homes and hostels in the western suburbs. There are currently 22 volunteer visitors and more volunteers are needed as the need is growing. Residents are matched with volunteers who can support their cultural, linguistic or social needs. Employment opportunities
Support workers are being sought by Wesley Home Care in all suburbs of the metropolitan area. All support workers are employed as multi-skilled workers to assist with the following tasks:

Domestic assistance that includes mopping, vacuuming, wet areas etc.

Personal care that includes, showering, grooming, dressing

Meal preparation

Transportation of clients for appointments

Respite
