Community Aged Care Packages

57 CACPs are managed from Direct Care Services providing care to people in the western region of Adelaide. Recipients of these packages must have a current Aged Care Assessment approval for Low Level Care.

These packages are planned and coordinated packages of care to help older people to remain living in their own homes. They are funded by the Federal Government, and have a mean-tested co-payment by the client. All C.A.C.P Care Plans are designed by the co-ordinator in conjunction with the client and their advocate. This allows and provides for flexibility in the design of each individual plan.

The Care Plans may include assistance with:

Personal Care such as:

  • showering, bathing or personal hygiene
  • Social support
  • Transport to appointments
  • Laundry
  • Meal preparation
  • Domestic support
  • Shopping and bill paying
  • Liasing with other service providers

Care Plans are reviewed and revised regularly to suit the clients ever changing needs For further information contact the C.A.C.P Co-ordinator on (08) 8346 5249.

Veterans Home Care

To access this service the enquiries must go through the Veterans Home Care Assessment Agency. Phone 1300 550 450.

Assistance with Care and Housing for the Aged
