
Uniting Care Wesley Bowden is an Australian Government Funded Program that offers Community Aged Care Packages (CACP). These are flexible health care and support packages which help people to remain in their own home when they would otherwise move to a residential care facility.
CACP's offer holistic care delivered in your home and tailored through negotiation to meet your needs. Uniting Care Wesley Bowden can also link you to other appropriate services and activities in the community.
To be eligible for a CACP you must live in the Western Metropolitan Region of Adelaide and you need to be assessed by a member of the Aged Care Assessment Team (ACAT). ACAT can meet with you in your own home or in hospital to determine whether a CACP is suitable.
Your local ACAT might include a doctor, nurse, social worker, occupational therapist or physiotherapist.

• Personal care such as bathing, showering or dressing
• Social Support
• Transport to appointments
• Laundry
• Meal Preparation
• Domestic Support
• Shopping and Account Paying
• Minor Home Maintenance and/or gardening
